NZQA Unit Standard 17601 - Produce an occupational health and safety incident investigation

This HSR course provides Health & Safety Representatives with the tools to be able to become an effective health & safety officer in the workplace. It builds on learning from Stage 1 and covers the key components of hazard management and incident investigation.

  • This Health & Safety Representative course is also useful for other levels within the organisation who wish to improve safety in the workplace by the use of sound safety systems.

Key Learning Objectives:

  • Achieve an understanding of hazards, significant hazards and serious harm in relation to Health & Safety at Work Act 2015.
  • How to identify hazards and implement control measures.
  • Learn how to monitor control measures for effectiveness.
  • Understand the difference between a hazard and a risk.
  • Purpose of the hazard register and its advantages in managing risk.
  • Understand legislative requirements of incidents and accidents.
  • Gives a holistic appreciation of incident investigation and prevention of re-occurrence.
  • Barriers to reporting near miss incidents and how to overcome them.
  • Front Line Leaders role immediately following an incident.
  • Understand the concept of multi causes of accidents and incidents.
  • Understand the various components of the investigation process including, post incident, data collection, analysis and prevention.
  • An awareness of human error principles.
  • How to conduct effective witness interviews.
  • Conduct a hypothetical incident investigation. (Team exercise).

Our health and safety representative trainers are from high risk industries and have vast experience in the practical use of hazard identification and risk management.  We conduct serious harm investigations using the ICAM tool for client organisations.
